When the novel coronavirus SARS-CoV-2 began spreading from northern Italy across Europe in early 2020, it struck a healthcare system that ranks among the most capable in the world — yet reached its structural limits within weeks. Lombardy in particular experienced one of the most severe humanitarian crises of the post-war era: intensive care units were repeatedly expanded, exhibition halls converted into emergency wards, and medical staff worked under extreme conditions amid severe shortages of protective equipment. Doctors and nurses on duty in Italy at the start of the pandemic faced a double burden — as caregivers and often as patients themselves. Hundreds of them died from the effects of COVID-19 infection in the first months of the pandemic. Italy was thus the first European country to reveal the full severity of this crisis.
Italy's healthcare system, the Servizio Sanitario Nazionale, was founded in 1978 as a universally accessible, tax-funded system and is one of the oldest of its kind in Europe. During the COVID-19 pandemic it came under political pressure, as years of budget cuts and regional infrastructure imbalances became visible. At the same time, the crisis highlighted the key social role of everyone working in hospitals, care facilities and outpatient services — often under precarious working conditions and with insufficient resources. With the issue of a 2-euro commemorative coin in 2021, Italy publicly honoured these professions and made a statement of societal recognition for their efforts during the pandemic.
